Our services are delivered on an intermittent basis according to a plan of treatment established by the patient/family, patient's physician, and home health staff. You may qualify for home health care services if you experience any of the following:

You have recently been discharged from a hospital, nursing home, or rehabilitation center

You are under the care of a physician

You require considerable effort to leave your home and your absences from home are short and infrequent

You have had a recent illness, injury or worsening condition which requires skilled nursing care, physical therapy, speech language pathology, or continuing need for occupational therapy
Primary Choice Home Care of Michigan is owned by a Registered Nurse with 42 years of experience and 15 years of knowledge in the home health care industry. Together with our experienced staff, we are well equipped to serve you and your family.
